About the Role:
As an ER Medical Coder – E/M, Injections & Infusions, you will play a critical role in supporting our healthcare clients by reviewing clinical documentation and accurately assigning diagnosis and procedure codes across a variety of outpatient settings. You will work closely with providers and coding teams to ensure coding accuracy, regulatory compliance, and productivity standards while delivering high-quality coding services for Emergency Department, outpatient surgery, observation, and diagnostic encounters.
Work Schedule/ location: Remote (Flexible Hours, Monday–Friday)
In this role, you will:
- Review clinical documentation and assign accurate ICD-10-CM diagnosis and procedure codes for outpatient surgery, observation, Emergency Department, and outpatient diagnostic encounters.
- Utilize electronic encoder applications to assign codes in accordance with company policies and regulatory guidelines.
- Communicate with physicians to obtain or clarify diagnoses and procedures through compliant query processes.
- Perform peer reviews of coding auditors and coders to support quality and consistency initiatives.
- Prepare and complete coding-related reports as requested.
- Maintain a minimum coding accuracy rate of 98% while achieving established productivity standards.
- Ensure compliance with federal, state, payer, and client-specific coding guidelines and regulations.
- Work effectively in a remote environment while managing multiple systems and priorities.
This is a remote position supporting one or more Cognizant healthcare clients. Candidates must be comfortable working independently, managing priorities, and collaborating with team members in a fast-paced environment.

What you need to have to be considered:
- One of the following certifications: CCS, RHIT, RHIA (AHIMA), CPC, or COC (AAPC).
- Minimum of 2 years of facility-based outpatient coding experience.
- Experience coding Emergency Department encounters, including facility Evaluation & Management (E/M) coding.
- Experience with injections and infusion coding in an outpatient or hospital setting.
- Knowledge of federal, state, and payer regulations related to medical coding and reimbursement.
- Ability to interpret medical records, reports, and chart documentation accurately.
- Strong analytical, judgment, and decision-making skills.
- Ability to work independently and as part of a collaborative team environment.
- Basic proficiency with Microsoft Word and Excel.
- Ability to navigate multiple systems simultaneously in a remote setting.
- Strong written and verbal communication skills.
These will help you stand out:
- Familiarity with electronic encoder applications and coding technology tools like Cerner, PowerChart, 3M HIM/3M Coding Systems is preferred but not required.
- Experience in performing coding peer reviews and quality audits.
- Excellent organizational skills and attention to detail.
- Ability to adapt to changing client requirements, procedures, and policies.
- Self-motivated approach with the ability to work autonomously in a fast-paced environment.
Hourly Rate and Other Compensation:
The hourly rate for this position is between $33.00 – $35.00 per hour depending on experience and other qualifications of the successful candidate. This position is also eligible for Cognizant's discretionary annual incentive program, based on performance and subject to the terms of Cognizant's applicable plans.
